Phillip L. Banasiak, 59, of Knox, Indiana passed away Friday, January 25, 2013. Funeral services will be Saturday, February 2, 2013 at 11:00 a.m. CT at the M.C. Smith Funeral Home in Knox. Visitation is Friday from 4:00-8:00 p.m. CT at the funeral home with a Fire Service Memorial at 7:00 p.m. Memorials may be given to the Bass Lake Fire Department Scholarship Fund. Phil Banasiak was the former Fire Chief of the Bass Lake Fire Department.
