The Pulaski County Sheriff’s Department investigated a fatal crash that occurred Friday, August 16 in the area of State Road 39 and County Road 200 South.
Police say it was discovered that a passenger car traveling west toward Francesville on County Road 200 South was operated by Cody Sarallo, 31, of Burnettsville who failed to yield to the right-of-way to a passenger truck traveling south on State Road 39 operated by Audrey Krooswyk, 40, of Valparaiso.
Sarallo was pronounced deceased at the scene, according to the report. Krooswyk and two juveniles in her car were treated and released with minor injuries after being evaluated by Emergency Medical Services.
Deputies requested assistance from the Indiana State Police Crash Reconstruction unit as part of the investigation.
No other information was released.
