
Officials at Pulaski Memorial Hospital encourage residents to complete a survey to help the hospital in its strategic planning for the next several years.
Every three years, Pulaski Memorial Hospital completes a Community Health Needs Assessment to identify and prioritize the community’s health concerns and the survey is instrumental in gathering community responses to the health needs of the community.

The survey is available online at www.pmhnet.com. The survey is also available at the admission desks in the hospital and all five clinic locations throughout Pulaski County and Starke County. Paper copies can be picked up at the Pulaski County Public Library and Pulaski County Human Services. The QR code pictured may also be used to access the survey.
The Community Health Needs Assessment survey is available until June 19th.
Once all of the surveys are collected and analyzed, a written report will be given to Pulaski Memorial Hospital with the results posted on the hospital’s website.
